As part of our research, you will be asked to participate in an online interview (via zoom, MS teams, or Google Meet) about your experience with BlueSprig Autism and other providers. The interview will take no more than 30 minutes. At the end of the fully completed interview, we will provide within twenty-four hours, a gift card (sent to your email address) in the amount of $200.
Participants’ Rights
- I understand that I may skip any questions that I do not wish to answer or complete, and my level of participation is my choice.
- I understand that I can ask questions of the interviewer to better understand what I am being asked about.
- I understand that my responses will be kept confidential and will be available only to the researchers at Useful Group and BlueSprig Autism.
- I understand that my participation is at-will and I can leave the interview at any time.
- I agree to have my interview videotaped and transcribed for further analysis. The recording will be kept confidential and will be available only to the researchers at Useful Group and BlueSprig (“the researchers”). The video of my interview will never be shown publicly nor to any persons other than the researchers.
- I understand that I may be discussing autism care and treatment of my child or children and my interaction with BlueSprig Autism. I agree that all information conveyed during the interview by me or BlueSprig is permissible under HIPAA and other patient-privacy laws and I provide that permission here.
I consent to the researchers contacting me by email or text message after my participation in this interview to seek clarification of some of the information revealed in the interview or provide information to me about autism treatment.
