Employee Consent for Company Text Messages (SMS Communications)

Bluesprig Pediatrics (“Company”) may use text messaging (SMS) to communicate with employees for work-related purposes, including but not limited to:

  • HR announcements, company-wide communications, and safety or emergency alerts
  • Benefits enrollment communications
  • Center closures due to weather or holidays
  • Onboarding communications for new employees
  • Company-wide surveys (e.g., engagement, satisfaction, or feedback surveys)
  • Exit surveys sent to staff following separation from employment
  • Standard message and data rates from your mobile carrier may apply.

Please review the options below and select your preference. You may change your preference at any time — see the “Changing Your Preference” section.

Consent Options

Consent Options

This consent applies to current employees and, where applicable, to former employees who receive a one-time exit survey text following separation from employment. If you opt out, the Company will not send you text messages either during or after your employment, and will instead use email, phone call, or mail for any required post-employment communications. If you are rehired by the Company, this consent form will be reissued and you will be asked to confirm or update your preference as part of the rehire onboarding process.

Changing Your Preference

You may change your consent status (opt in or opt out) at any time by emailing [email protected] with your name and your requested preference. Your updated preference will take effect as soon as administratively possible after the request is received. Please ensure your mobile phone number in your ADP personal profile is kept current, as this is the number the Company will use to send text messages.

Annual Re-Consent

Each year, the Company will ask you to reconfirm your text messaging preference. If you previously opted in, you will receive a text message asking you to confirm or change your preference and will have 7 days to respond; if no response is received, your prior preference will remain in effect. If you previously opted out, you will receive an email asking you to opt in or opt out; if no response is received, your prior preference will remain in effect. Changing your mobile phone number does not reset your consent — consent follows you as the employee, not a specific phone number.
